PADDOCK TORONTO — MINOR PARTICIPATION: PARENT/GUARDIAN CONSENT AND RISK ACKNOWLEDGMENT

For a participant under 18. A parent or legal guardian must personally read, complete and sign this form. A child, friend, sibling or other accompanying adult must not sign unless they are the participant's legal guardian.

1. Participant, authority and covered session


I am the parent or legal guardian of the participant named below and have legal authority to give permission for their participation. I give permission for them to take part in the Paddock Toronto simulator session identified below at 105 Gordon Baker Rd, Unit 105, North York, ON M2H 3P8.


This consent covers that session, its safety orientation, and getting into and out of the simulator. It does not automatically cover later bookings or unrelated activities.

2. Risks to understand before deciding


Sim racing uses physical equipment even though the race is virtual. Direct-drive steering and force-feedback pedals may produce strong, sudden or unexpected forces. A steering wheel may turn quickly, kick back or resist movement. Fingers, hands, wrists and arms can be struck, twisted, trapped or strained; injuries can include cuts, sprains, dislocations and fractures.


Pedal resistance, moving parts and loose hair, jewellery or clothing can cause pinching, entanglement or injury. Other risks include fatigue, pain, slips or trips, and losing balance while entering or leaving a rig. Dizziness, nausea and motion sickness can occur even though the rigs are stationary.


Equipment or software faults, unsuitable setup or mistakes by participants or staff may also cause harm. Some injuries may be serious or lasting. Lower force settings do not remove all risk.

3. Orientation and safe participation


Before starting, staff must assess whether the participant can fit the rig, reach and operate its controls safely, and understand the safety instructions. Staff may decline or stop participation if it cannot be carried out safely.


Staff will demonstrate the stop button on the right side of the simulator, the stopping procedure and force controls. The participant must not start until they understand the instructions, and should ask staff for help choosing and adjusting a manageable force level. Increasing force may increase strain and injury risk.


I will explain these risks and instructions to the participant in a way they can understand. The participant must follow staff instructions, secure loose hair, clothing and jewellery, keep food and drinks away from equipment, and avoid bypassing safety features or interfering with another rig. They must not try to stop a freely rotating wheel by grabbing it.


The participant may stop at any time. If they feel pain, excessive force, dizziness, nausea or discomfort, or notice a fault, they should use the demonstrated stopping procedure and alert staff. They should ask for assistance whenever needed. Signing this form does not replace the safety orientation.

4. Meaning of this consent


I understand the risks described above and give permission for the named participant to take part, subject to the safety requirements. This form records my permission and acknowledgment of risks. It does not purport to waive the participant's legal rights or release Paddock Toronto from liability, and does not require me or the participant to indemnify anyone.

5. Privacy and records


Paddock Toronto will collect the participant's name, my name and parent/guardian role, my email, the session details, my confirmations and signature, the form version and submission date/time to record permission, administer the session, provide a copy on request, and address relevant legal or insurance matters.


The form records will be processed and stored using Wix and its service providers. Access is limited to authorized personnel and providers who need the records for those purposes. Relevant records may be provided to professional advisers, insurers or authorities when necessary for a claim, legal obligation or other lawful purpose.


Records will be retained only as long as reasonably necessary for those purposes, taking account of applicable legal requirements and insurance needs, then securely deleted or anonymized. Do not enter the child's email, phone number, full birth date, health information or identification documents. This consent does not include marketing, photography or video permission.

6. Personal signature by parent or legal guardian


I have had the opportunity to read this form and ask questions before signing. By personally typing or drawing my signature, selecting the electronic-signature confirmation and submitting, I intend to sign this exact version electronically. The participant must not complete the signature for me.
